State Testing Opt Out

 If you’d prefer your child to NOT participate in state testing, please fill out the forms below by making a copy and either emailing, or drop them off, to front office.  Hard copies are also available in the office.

Below are the grades and subjects tested and the forms (one is required to opt out of the LA/Math portion and the other is to opt out of the Science portion).  

7th Grade

OSAS Language Arts and Math Opt-Out Form

*There are NO testing requirements for 9th, 10th or 12th Grade.
